The information requested in this form is essential to TaxAidCenter.org’s efforts to verify you are who you claim to be, to locate your data within our systems, and to determine whether we may have a lawful reason to deny your request. The California Consumer Privacy Act (“CCPA”) came into effect on January 1, 2020 and it reinforces the protection of personal information for California residents.
